Furosemide is a loop diuretic that interferes with the transport of Na+ and Cl- in the thick ascending limb of the loop of Henle. In addition to inhibiting the reabsorption of these ions and potassium in this portion of the nephron (by acting on the Na+/K+/2Cl system), it also reduces the reabsorption of sodium and chloride in the distal convoluted tubule. Diuresis results from the increased excretion of ions that would maintain the hypertonicity of the interstitium – which causes the passive reabsorption of water along the nephron.
Furosemide also has vasodilatory properties, increasing renal perfusion and leading to cardiac preload. In addition, it can be administered in a wide range of doses and can be used in chronic administration or emergency therapy.
